Picking up where the Outreach Specialists leave off, our Case Managers work with clients coming off the streets and into emergency housing to assess their health and social needs and identify barriers and strengths to obtain transitional and/or permanent housing. If the person is receptive to emergency shelter, our team transports them to shelter and ensures they have a follow-up appointment with our intensive Case Management Team the following business day. Our Outreach Specialists distribute food and emergency items to individuals, initially triage service needs, and gauge an individuals’ willingness for emergency housing placement. The process starts with engagement by our professional Outreach Specialists and supporting volunteers to persons living on the street or other places not meant for human habitation. Provides services to all Essex County Residents. We pride ourselves on working closely with employers and building upon our database of employers so that we are able to find the best jobs to meet the needs of those we serve. Our placement specialists have more than a combined 35 years of locating suitable employment for disadvantaged individuals. We put people to work! We help clients choose, get, and keep a job. Will coordinate their activities with designated agency representatives, e.g., Reentry Specialists and Parole to help assist the participant with adherence to supervision requirements andĬase managers will conduct comprehensive job readiness services focused on enhancing skills needed to sustain employment. Will oversee and monitor the progress of the client to ensure that the identified service goals and treatment are adhered These services may include, but not limited to mental health, substance abuse, legal services, and obtaining government issued identification Will link participants with appropriate resources in the community so that extensive barriers are addressed and remediated. Have a knowledge-base of available community resources in the event a client is in need of extensive services beyond employment By working with our clients, we are able to allow the client to remain in the driver’s seat, so to speak, in identifying and reaching his or her vocational goal. Our collaborative process with our clients assesses, plans, implements, coordinates/links, monitors, and evaluates the many options available to meet the expansive needs of our clients. In addition, the VAST will assess medical factors, educational background, along with work preferences, strengths, values, skills, abilities, level of motivation to work and potential barriers, in order to determine the client’s level of work-readiness. The VAST, administered through structured conversations, will appraise the individual’s work/training background, general functional capacities, and social/behavioral behaviors. VOCATIONAL ASSESSMENT SCREENING TOOL (THE VAST) Our Primary Objective is To ensure the economic playing field is leveraged for low-income, hard –to-place urban populations, such as citizens returning from incarceration, substance abusers, TANF and SNAP recipients, as well as at-risk youths, by providing access to comprehensive, relevant job skills training, leading to immediate job opportunities, and long-term career opportunities. Our mission is to help individuals achieve self-sufficiency to promote wellness sand community development through employment, education, and housing.
